Information about issuance of results:
The results were released in eleven (11) working days from the last day of examination. Registration for the issuance of Professional Identification Card (ID) and Certificate of Registration will start on Monday, July 18, 2005 but not later than August 12, 2005.Those who will register are required to bring the following: duly accomplished Oath Form or Panunumpa ng Propesyonal, current Community Tax Certificate (cedula), 2 pieces passport size picture (colored with white background and complete nametag), 1 piece 1” x1” picture (colored with white background and complete nametag), 2 sets of metered documentary stamps, and 1 short brown envelope with name and profession; and to pay the Initial Registration Fee of P600 and Annual Registration Fee of P450 for 2005-2008.
Successful examinees should personally register and sign in the Roster of Registered Professionals.
Oath-taking
Date of Oath-taking ceremonies: August 15, 2005
Time: 8:00 a.m.
Venue: Araneta Center, Cubao, Quezon City
in Iloilo, Baguio and Cebu City on August 17, 2005; in Cagayan de Oro City on August 18, 2005; in Davao and Tacloban City on August 19, 2005; in Tuguegarao and Zamboanga City on August 22, 2005; in Legazpi City on August 23, 2005. Please contact the Accredited Professional Organization (Philippine Nurses Association) local chapter for further details.
Attire Guidelines:
1. while gala uniform
2. nurses cap
3. white duty shoes
4. without earrings
5. hair not touching the collar
6. without corsage
Tickets will be available starting Monday, July 18, 2005 at the Ground Floor, PRC –Main Building, Sampaloc, Manila.
Requirements for registration for oath-taking:
